Welcome, my name is Marie Smith and I am the school nurse at Stockton School. 

I am excited to welcome our students to Stockton School for the current school year. I hope that all of you are staying safe and following the latest guidelines regarding Covid-19.  

School is very different from what we were used to. Masks are now optional unless you have tested positive. Any positive cases must stay home for 5 days and wear a mask for days 6 through 10. They will also eat in the conference room for days 6 through 10. I am available by phone at 856-424-1505 X 2 or by e-mail at msmith@chclc.org. I check my e-mail frequently and will get back to you with any questions or concerns.

I have included some helpful information on finding medical forms below. Please contact me if your child has any medical conditions.

Many health forms are available on the Cherry Hill District Website.

1. Go to the C.H. District website

2. Click on the Departments Tab

3. Choose "Health Services" from the drop down menu

4. Choose the form you need

FYI - The tylenol/advil parent permission form is also available in Genesis and you may complete it there.
