If your child has a life threatening allergy and may require the administration of Epinephrine and/or Benadryl while at school, the following forms are required. Please complete the Administration of Epinephrine form and a Medication administration for for each medication  

Medication will not be accepted in the nurse’s office without the proper completion of the below forms. All medication must be in the original containers.  Epipens should not be expired and must have a prescription label which matches the doctor's order.

A physical must be conducted and recorded by your physician or nurse practitioner on the New Jersey State Pre-Participation Physical exam form within 365 days of the sport's first day of practice.
Please have the form completed.
