- Fifth Grade "Graduates" Health Requirements

Cherry Hill School District Policy and NJ State Law mandate that all eleven year old and older students entering sixth grade must receive meningitis (Menactra) and pertussis containing (Adacel or Tdap) vaccines. Students who are eleven years old and do not present documentation of having had these vaccines will be excluded from school on the first day in September and will be until such documentation is received. If a child's check-up is scheduled for a date after the first of September, or their birthday occurs late in the calendar year, you can submit a copy of the appointment card or a document from the healthcare provider indicating the date and time the student has an appointment.
If a student is anticipating participating in athletics in middle school he/she will be required to have an athletic physical exam recorded on the approved state form. You can submit both the immunization record (or the document of a pending appointment) and the athletic physical exam report to the middle school nurse after June 1 of the fifth grade year, or submit to me before June 1.-
PLEASE NOTE: THE SPORTS PHYSICAL CLEARANCE PROCESS HAS CHANGED EFFECTIVE JULY 1, 2024. Please read all the information below carefully.
THE New Jersey Department of Education (NJDOE) HAS CHANGED THE SPORTS PHYSICAL SUBMISSION PROCESS. THE SPORTS PHYSICAL EXAMINATION FORM NO LONGER NEEDS TO BE SUBMITTED TO THE STUDENT’S SCHOOL DISTRICT. A MEDICAL ELIGIBILITY FORM will still be required. The Medical Eligibility Form should be in the ORIGINAL form. Please do NOT EMAIL. The Medical Eligibility Form for FALL sports is due to the nurse's office by August 12, 2024.
New Jersey State Law requires that all students that plan to participate in the after school Athletic Program in Middle School and High School have a physical exam. **THIS EXAM IS TO BE KEPT BY THE STUDENT’S HEALTHCARE PROVIDER WHO COMPLETES THE PREPARTICIPATION PHYSICAL EVALUATION.
- Preparticipation Physical Exams are valid for 365 days. The physical must be current on the day of the first practice to be considered valid. Please be aware that physical exams are recorded based on the date of exam, not the date the form was completed by the physician.
- Sports Physical Exams must be recorded on the New Jersey Department of Education Annual Athletic Pre-Participation Evaluation Form. THIS FORM IS TO BE KEPT ON FILE BY THE STUDENT’S HEALTHCARE PROVIDER.
- THE MEDICAL ELIGIBILITY FORM MUST BE COMPLETED BY THE HEALTHCARE PROVIDER, AND SUBMITTED TO THE SCHOOL DISTRICT. THIS IS THE ONLY FORM THAT IS REQUIRED TO BE SUBMITTED TO THE NURSE’S OFFICE.
EACH SEASON:
□ Verify that your child’s physical examination is up to date and that you have submitted the MEDICAL ELIGIBILITY FORM to the nurse’s office.
□ Complete the Health History Update Questionnaire in the FORMS SECTION of the Genesis Parent Portal.
□ Complete additional release forms via the Parent Genesis Portal. Review and electronically sign the release forms each season during the registration process.
- Review the Opioid Use/Misuse Fact Sheet and video with your student athlete and electronically sign the acknowledgment form in the Genesis Parent Portal each season.
- Review the Sudden Cardiac Death in Young Athletes Pamphlet with your student athlete and electronically sign the acknowledgment form in the Genesis Parent Portal each season.
- Review the Sports-Related Concussion and Head Injury Fact Sheet with your student athlete and electronically sign the acknowledgment form in the Genesis Parent Portal each season.
- Complete the Athletic Participation and Release Form, select the sport for that season and electronically sign the acknowledgment form in the Genesis Parent Portal each season.
