Kindergarten Registration Presentation
All students entering Thomas Paine are required by either district policy and/or NJ state law to have all of the following documents:
1. A valid immunization record that includes ALL of the following immunizations:

DTaP (Diphtheria, Tetanus, Pertussis)

Any 5 doses or any 4 doses with the last dose given after the 4th birthday.

IPV (Polio)
-
Any 4 doses or any 3 doses with the last given after the 4th birthday.

MMR (Measles, Mumps, Rubella)

Two doses, the first given after the first birthday
-
-
Varicella (Chicken Pox)
-
One dose given after the first birthday (2 is suggested)

HBV (Hepatitis B)
-
Three doses

** Please note that exemptions can be made for students who provide either:

A written statement signed by a parent or guardian explaining how the administration of immunizing agents conflicts with the pupil's exercise of bonafide religious tenents or practices. General philosopical or moral objection to immunizations shall not be sufficieient for exemption.

A written statement from a physician stating that a specific immunization is medically contraindicated.
2. A health physical that has been completed and signed by a physician within 365 days of the pupil's registration to school.
3. An updated health history record that has been completed and signed by a parent or legal guardian.
4. An emergency contact form with valid points of contact that can be reached should your child encounter an emergency situation while at school.
