The Food Allergy Plan (attached) is the form that must be completed if a child has an allergy and might need  to receive emergency medication at school (i.e.Benedryl, Epi-pen).  The parent must complete the emergency contact information and sign the form.  The physician must complete the treatment section and sign the form.
