Important Program Details

The program is fully administered by VSP Vision^TM Eyes of Hope®, a charitable initiative of VSP Vision^TM.

A Social Security number (SSN) is not required to use gift certificates provided by the program. Certificates include a pre-populated Patient ID that the doctor's office will use to administer the covered benefits.

For a child to qualify for the program, they must meet the following criteria:

• Family income is at or under 200% of federal poverty guidelines.
• Does not have vision coverage.
• Can only use this program once every 12 months.
• Letter to parent/guardian about vision care gift certificates
