The New Jersey Dept. of Education recently updated the Preparticipation Physical Evaluation Form for students entering grade 6. The old form will not be accepted after 06/30/2024. Please navigate to the school health forms tab to download the new forms at the bottom of the page, if your student would like to participate in athletics.
